予約について

当院では、外国籍の方の検査・治療もお受けしております。

予約方法

ご本人もしくは旦那様(男性パートナー)が日本語でのやり取りが可能な場合は、通常どおりお電話かLINEかWEB予約システムよりご予約をお取りください。

通訳について

安全かつ的確な治療のためにご協力をお願いいたします。
当院では、国籍に関係なく、日本語で医師・看護師とのコミュニケーションが問題なく行える方については、通訳の同行は不要です。
一方で、言語にご不安がある方は、ご自身で医療通訳のご手配をお願いいたします。

医療通訳の手配について

当院には外国語に対応可能なスタッフもおりますが、すべての診察に毎回対応できるわけではありません。
医療通訳をお願いする際は、翻訳機やご家族・ご友人による通訳ではなく、専門の医療通訳者をご手配ください。
手術や処置時にオペ室への立ち入りが必要となることがあるため、通訳者は必ず女性の方でお願いいたします。

通訳が不要となる例外

以下に該当する場合は、専門通訳者の同行は不要です：

旦那様（男性パートナー）が日本国籍をお持ちで、日本語による説明を毎回の診察時に同席して受けられる場合
受診時からお会計までお付き添い頂く必要があります

通訳が必要な場合の
追加費用について

通訳を介した診療では、説明に通常より多くの時間を要するため、所定の手数料がかかります。詳しくはお問合せください。
なお、日本語をある程度話せる方でも、治療内容の正確な理解が必要と判断される場合には、当院より女性医療通訳者の手配をお願いすることがございますので、予めご了承ください。

日本の公的医療保険を利用される方へ

◆保険診療における多言語診療サポートについて

当院では、安全かつ適切な医療を提供するため、日本語のみでは診療内容について十分な理解・確認が難しく、通常より説明に時間を要する場合や、中国語等による補足説明・確認などの多言語対応が必要となる場合があります。

2026年度診療報酬改定において、在留外国人の診療に必要となる多言語対応に要する費用について、保険診療とは別に徴収できることが示されました。

これに基づき、当院において多言語診療サポートが必要と判断され、実際に多言語での説明・確認等を行った受診日については、保険診療の一部負担金とは別に、多言語診療サポート料として5,500円（税込）／受診日を頂戴いたします。

なお、日本語での日常会話が可能な場合でも、医学的な説明について十分な理解・確認が難しく、多言語による補足説明等が必要な場合には対象となることがあります。

あらかじめご了承くださいますようお願いいたします。

持ち物

身分証明書
（必須）

日本の健康保険証を(もしくは保険証登録済みのマイナンバーカード)をお持ちの方はご持参ください。
当月初めての方は、保険証をご提示いただいております。確認がとれない場合は、一旦自費（10割負担）でお支払いただくことになりますので、予めご了承ください。
保険証やマイナンバーカードがない場合は、名前・生年月日が確認できるもの（運転免許証やパスポートなど）をご持参ください。

問診票（必須）

問診票をダウンロードしてご記入いただき、受診当日に持参してください。
日本語での記載が難しい場合は、通訳の方が代理でご記入ください。

治療方針を決定する際に重要となるため、可能な限り詳細に記入するようにお願いいたします。
A4サイズの用紙に印刷し、ご記入いただいたものを来院時に受付へ提出してください。
当院では、初診時に男性の方にも健康状態を確認するために必ず採血検査をお受けいただいております。初診でのご来院の際は、可能な限りお二人でご受診下さい。
問診票を事前にご記入いただくことが難しい場合は、ご予約時間の30分前にご来院いただき院内でご記入をお願いいたします。

紹介状

他院の紹介状がある場合は初診時にお持ちください。
外国語で書かれた紹介状の場合必ず日本語で翻訳したものも一緒にお持ちください。

基礎体温表

つけているものがあればお持ちください。
基礎体温表はこちらのページからダウンロードいただけます。

検査結果

1年以内に受けた感染症検査(クラミジア・梅毒・B型、C型肝炎・HIV)の結果をお持ちでしたら検査結果用紙の原本をご持参ください。

検査結果をお持ちいただく際は、こちらのページから「検査データの持ち込みについて」をダウンロードし、「採血項目チェックリスト」を印刷してご記入いただき併せてお持ちください。

法律婚の方

【カップルのどちらかが外国籍の場合】
日本国籍の方：戸籍謄本（全部事項証明／発行から3ヶ月以内）
外国籍の方：婚姻関係を証明する書類（以下のいずれか）
　 ∟婚姻内容が記載された住民票
　∟大使館・領事館で発行された婚姻証明書

【カップルの両方が外国籍の場合】

婚姻関係を証明する書類（上記いずれか）
特別永住者証明書または在留カード
事実婚の場合：独身証明書（3ヶ月以内）
※必要書類の取得にはお時間がかかる場合がありますので、早めのご準備をおすすめします。

事実婚の方

①同意書
『事実婚カップルへの同意書』にそれぞれ直筆でサインしたものを初診当日ご提出いただきます。こちらのページから事前にダウンロードをしてご準備いただくことが可能です。

②独身証明書

治療開始時までに別途それぞれの「独身証明」（3ヶ月以内に発行された原本）が必要です。
※但し日本国籍の方は戸籍抄本をご用意ください。
移植周期に入る場合、独身証明書もしくは戸籍抄本の提出が必要ですが、前回提出した日から1年以内の方は再度提出する必要はございません。

About Medical Interpreters

For your safety and to ensure accurate treatment, we kindly ask for your cooperation.
At our clinic, patients of any nationality who are able to communicate smoothly in Japanese with our doctors and nurses
do not need to be accompanied by an interpreter.
However, if you are not confident in your Japanese ability, please arrange a professional medical interpreter yourself.

Arranging a Medical Interpreter

Our clinic has staff who can communicate in foreign languages; however, they may not be available for every consultation.
If you require interpretation services, please arrange for a professional medical interpreter rather than using a translation device or relying on family or friends.
For surgeries or procedures, the interpreter may need to enter the operating room, so we request that the interpreter be female. The interpreter is expected to accompany the patient throughout the visit, from consultation to payment.

Exceptions Where an Interpreter Is Not Required

An interpreter is not necessary in the following case:

Your husband (male partner) is Japanese and can attend every consultation with you to receive explanations in Japanese.

Additional Fees for Interpreter-Assisted Consultations

When an interpreter is required during a consultation, additional time is needed for explanations, and an additional fee may apply. Please inquire for more information.
Please note that even if you can speak some Japanese, our clinic may still require you to arrange a female medical interpreter if we judge that accurate understanding of your treatment is necessary. Thank you for your understanding.

Information for Patients Using Japanese Public Health Insurance

◆Multilingual Medical Support for Patients Covered by Japan’s Public Health Insurance

To ensure safe and appropriate medical care, some patients may require additional time for explanation or multilingual support, such as supplementary explanations and confirmation in Chinese or another language, when medical information cannot be sufficiently understood or confirmed in Japanese alone.

Under Japan’s 2026 medical fee revision, medical institutions may charge separately for multilingual support required in providing medical care to foreign residents covered by Japan’s public health insurance.

When our clinic determines that multilingual medical support is necessary and such support is actually provided during a visit, a Multilingual Medical Support Fee of JPY 5,500 (tax included) per applicable visit will be charged separately from the patient’s regular co-payment under Japan’s public health insurance.

This fee may also apply to patients who can communicate in everyday Japanese if additional multilingual explanation or confirmation is required to ensure sufficient understanding of medical information.

Items to Bring

Identification

If you have a Japanese Health Insurance Card (or a My Number Card registered with your insurance), please bring it with you.
If this is your first visit of the month, you will be asked to present your insurance card.
If you cannot present your insurance card, you will be required to pay the full amount (100% out-of-pocket) at the time of consultation. Please understand this in advance.
If you do not have a Health Insurance Card or My Number Card, please bring an official document that verifies your name and date of birth (such as a driver’s license or passport).

Medical Questionnaire

Please download and complete the medical questionnaire, and bring it with you on the day of your consultation.
If it is difficult for you to fill it out in Japanese, your interpreter may complete it on your behalf.

Since this information is essential in determining your treatment plan, please fill it out as thoroughly as possible.
Print the questionnaire on A4-size paper and submit the completed form to the reception upon arrival.
At our clinic, all male patients are required to undergo a blood test at their first consultation to assess overall health status. We recommend that both partners come together for the initial visit whenever possible.
If you cannot complete the questionnaire in advance, please arrive 30 minutes before your appointment to fill it out at the clinic.

Referral Letter

If you have a referral letter from another medical institution, please bring it to your first consultation.
If the referral is written in a foreign language, you must also provide a Japanese translation.

Basal Body Temperature Chart

If you have been recording your basal body temperature, please bring your chart.

Test Results

If you have the original test results (within the past year) for infectious disease screening (Chlamydia, Syphilis, Hepatitis B/C, HIV), please bring them.

For Legally Married Couples

If one partner is a foreign national:
Japanese partner: Family Register Certificate (Koseki Tohon, full version, issued within the last 3 months)
Foreign partner: Document certifying marital status (one of the following)
∟ Certificate of Residence (Juminhyo) showing marital details
∟ Marriage certificate issued by an embassy or consulate

If both partners are foreign nationals:
Document certifying marital status (as above)
Special Permanent Resident Certificate or Residence Card

For common-law marriages: Certificate of Single Status (issued within the last 3 months)
Please note that obtaining the required documents may take time, so we recommend preparing them as early as possible.

For Common-Law Couples

1.Consent Form
Both partners must sign the “Consent Form for Common-Law Couples” by hand and submit it on the day of the first consultation.
The form can be downloaded in advance.

2.Certificate of Single Status
Both partners must prepare an original “Certificate of Single Status” (issued within the last 3 months) by the time treatment begins.
Japanese nationals should instead submit a Certificate of Family Register (Koseki Shohon).
For embryo transfer cycles, you must submit either a Certificate of Single Status or a Koseki Shohon.
However, if the previously submitted document is still valid within one year from the date of issue, re-submission is not required.
